How do I create a new consent form?
Go to Journal in the dashboard and choose to create a new template. You can either build your form from scratch or import a ready-made template from the library. The library contains predefined forms for common treatments (e.g., HIFU, Profilo). Imported templates are fully editable so you can customize them as needed.

Can I customize the questions in a consent form?
Yes. Some fields are always required. In addition, you can enable optional standard fields and add your own questions under Manage Questions. You can also require the customer to upload one or more pictures—for example, front and side photos of the face for injection treatments.

How can I see what the form looks like for the customer?
Use the preview feature in the form editor to see exactly what the customer will receive and fill out.

How do I link a consent form to a specific treatment?
You add and remove consent forms on the treatment, not by deleting the template in Journal. Edit each treatment where you want to make the change.

You can edit treatments by going to Services, editing a service, and opening the Treatments tab. Or go to Services and open the All Treatments tab (/dashboard/services/treatments). That table also shows which treatments have a consent form linked.

Open the treatment and select the form under the consent settings. From then on, the form is sent automatically to customers who book this treatment—whether they book online or you add the booking in the dashboard.

How do I remove a consent form from a treatment?
This is the reverse of adding it. Edit the treatment (via Services → edit service → Treatments tab, or via Services → All Treatments) and set the consent form to Consent not needed. Do not delete the consent form template itself to remove it from a treatment.

When does the customer receive the consent form?
The customer receives an SMS with a link to the form 48 and 24 hours before the appointment. In the booking view, you can see whether the customer has completed the form and resend the link if needed.

What happens if the customer does not complete the consent form?
If a required form is not filled out, this will be indicated in the calendar. The system will send reminders to the customer. Once the form is completed, this will be reflected in the calendar along with the customer’s responses.

How often is the consent form sent to the customer?
By default, each consent form is valid for one year—the customer will not have to fill it out again until that period has passed. This setting is configurable: you can set a shorter time frame, require the form before every treatment, or allow the doctor to request more frequent updates. We recommend you inform your customers in the form text that they must notify you of any significant changes in their health status.

What if the same consent form is used for multiple treatments?
The customer will only receive the form once within the validity period—it won’t be sent repeatedly without reason.

Can we require a doctor’s signature on consent forms?
Yes. You can enable doctor signature for a treatment. The responsible doctor will automatically be notified when new consent forms are awaiting signature. The doctor logs in to the same system, reviews the form, and signs it digitally. All signed forms are stored automatically in the patient’s journal. Once the doctor has signed, the customer is shown as ready and approved in the calendar view.

Can I register multiple responsible doctors?
Yes. You can have multiple responsible doctors in the system. They will be automatically notified whenever there is a consent form waiting for review and signature.

Do consent forms support multiple languages?
Yes. Customers receive forms in their own language. The system supports Norwegian, English, and Spanish.
